Gavin is such a great helper. He is always willing to get me a diaper or a pacifier for Cooper, or just hold him so I can take a much needed shower. I knew he would be great, but he is way better then I anticipated.

Parker loves Cooper and especially loves to hold him, even to my surprise. Whenever I am in the other room and Cooper starts to make the littlest cry, Parker immediately picks Cooper up and walks him to me. It freaked me out the first time...but I have to say he does pretty well.

Santa came!

Stocking were a little too heavy to hang. :)

Gavin insisted on leaving Santa a healthy snack, apple and berries with a cold glass of milk. He said Santa is too fat and needs to eat better. Poor kid! I hope Kyle and I being fitness freaks is only leaving a positive impression? And carrots and "magic reindeer food" for the reindeer's.

Santa brought very educational presents this year...yipee, no more junk!

Cooper's 1st Month

1 week old, 9lb 14 oz

We are loving our little Cooper. He is quite the chunk, I guess Mama's milk is doing his body good. He is a very content baby, he really only makes a fuss if he is hungry or has a burp or gas. It is easy to fix him and make him happy again. He just started smiling, pretty much on his 1 month birthday. He is making eye contact and is very attentive. He loves to be held and constantly switched around to different positions. Cooper is also a pretty good sleeper at night. He has a long snooze from 8pm to 1am and then from there on is hungry every 3 hours, like clock work. We can not get enough of him, especially the boys. They are such good helpers and are always eager to hold him. Life is great and we feel so blessed to have our 3 healthy boys.

At the hospital for my scheduled induction. After being 11 days overdue the doctor was concerned that the placenta was getting too old and could result in infection and lack of nutrients to the baby. As much as I wanted to wait for him to be ready and come out...I knew it was time. Gavin and Parker were both over due and labor was fast with them, so I was a little concerned and nervous to see how my body would respond to pitocin.

Kyle's artwork. He was a little nervous and bored. A lot different then my 3 hour labors with Gavin and Parker.

At 1pm we started pitocin. Now begins the waiting game. (talking to Alison, my twin!)

I was anxious to get moving and walk around, hoping labor would progress with a little help from gravity. Then just a few minutes after this picture was taken, Cooper's heart rate dropped from 140 to 50. It lasted for 5 minutes. 4 nurses came rushing in and then comes my doctor. They gave me a shot to immediately stop my uterus from contracting and stopped pitocin. The baby needed to relax after being under so much stress. So from then on (4pm) I was to stay on my side, only my right side (we tried the left and his heart rate started to fall again), until the end. Cooper had his cord around his neck, which we have known from the 20 week ultrasound, so standing and laying of my left side must have been putting pressure on the cord resulting in a lack of oxygen. A few hours later I really needed to go pee, so I got up real quick and peed...low and behold his heart rate dropped again. Ahhh! My doctor was very concerned. She wanted me to get an epidural to get numb and ready just in case his heart rate dropped again, because if it did, we would have to rush to do an emergency c section. Stubborn me, I did not want to do an epidural, so I asked if we could just wait a little and see how he does. She then broke my water at 7pm and we waited to see if labor would progress. Up until now I remained dilated at a 3. A little frustrating. I am having some good contractions but I am not dilating! After she broke my water she came to check on me at about 10pm. She checked me out and I was now a 6. Yipee, finally progress. Until she felt his hand over his head shooting out like superman. She attempted to move it, but no luck. Now we new it was epidural and c section time. 2 of the things I never wanted to hear. But, poor little Cooper was not coming out the "right way". Because of his cord and his arm he was not descending and was not putting enough pressure on the cervix to dilate it, so I would not have been able to deliver vaginally. A little disappointed for sure, I knew this was the right thing and I felt peace about it. I am about to meet my baby I have been carrying for 41 and a half weeks. So exciting!

Here is the little buddy. It took him about 30 seconds to make his first cry. Kyle said he looked like a smurf, totally blue. :( Finally I heard him make his first cry. I immediately burst into tears. This unexpected labor and delivery was finally over, and our baby is here safe and sound.

Family Hike at Kit Carson Park

It was a beautiful day so we decided to take advantage of it. We went to Kit Carson Park in Escondido. It has a really cool statue garden with all of these amazing, colorful statues. We had a lot of fun exploring and playing. Still no baby. 41 weeks pregnant and still with child!

I am ready for this little one to come! 2 days away from my "due date"...but we all know babies come when they are ready, so patien me is just enjoying the waiting game. Here are a few pictures of his room.

This nameless babies monogram will be finished when we finally decide on a name. I have my name and Kyle has his. Only time will tell to see who wears the pants in this family.

Bates Nut Farm

Kyle was on a double 24 hour shift and I wanted to do something fun with the boys, so we went to Bates Nut Farm in Valley Center. There are so many things to do so it was hard for the boys to decide. We started with rock climbing. Gavin made it almost to the top and Parker gave it his best effort. Then we got their faces painted, conquered the maze, slid down bouncy slides, and created a spin art frizbes. So much fun!

We went to Grandma and Papa Carnohan's for trick-or-treating. Such a great neighborhood. This year, one of the houses had a haunted house in the back yard. Gavin really wanted to do it, so with a little twisting of Daddy's arm, they did it. He came out loving it, and I am happy to say he has not had one scary dream. What a relief. They had a great time and got just enough candy.

36+ Weeks Prego

I am starting to really be over this pregnancy. I am feeling uncomfortable and huge. I am sick trying to be tough and hardcore. Sometimes Mommy needs to be babied too. Sometimes I need dinner ready for me and a little extra love. I know my hormones are going crazy and I am being a little bit of a crybaby, but hey my life is going to completely change in just a few weeks. I will have to push a watermelon out of my va-jay-jay and devote the next year of my life to nursing and having to be the sole provider to a completely dependent baby. I am excited and I can't wait to welcome a new baby into our family, but I am scared! Am I ready to give up just having two kids to now having 3? Not really. I am super nervous and worried I will not be able to give all 3 kids the love and attention they deserve. I feel like I don't have my 2 kids completely under control and I worry they will feel replaced by a very needy baby brother. I am trying to savor these last few weeks I have with just my Gavin and Parker. I am trying to do a few extra things with them and take in every last day I have with just them. I realize I am a little dramatic but that is how I am feeling right now. I may feel different in a few minutes.: ) I guess that's just being super pregnant!

This little baby is growing and all ready in position for his big escape. I had to have another ultrasound a few days ago because they were worried he was small based on the size of my belly. But I guess teaching 12 fitness classes a week can make your tummy measure small. :) He weighed in at 6lb 11 oz based on the ultrasound measurements, so with a little over 3 weeks to go he is not going to be too small. Thank goodness.

Here is to a few more weeks of having a gigantic belly!

I love overhearing their conversations they have together in the play room and the endless imaginative play they come up with. On Sunday I got struck with the stomach flu. I was feeling kinda sick on Saturday while we were in the OC, but Sunday during church I really felt it. I went home and with in an hour threw up 2 times. And then progressed to 4 or 5 more times on Sunday. It must have been pretty violent hacking for me to tear my right abdominal muscle. Which has caused me to have a super difficult time standing up, breathing, getting up etc. It is the worse, especially when I get muscle spasms that just won't quit. I am hoping it heals quickly because big Mama needs to get back to work at the gym and at home. Kyle has been amazing taking care of everything while I groan and complain. Heres to a speedy recovery.

OC Saturday

Thanks to my SIL Brooke for always taking pictures when we get together so there is proof of our fun time together. :) Kyle had an interview with OC Fire on Saturday, which went really well, we are anxiously waiting for the results! So we all drove up together and after the interview grubbed at Pick Up Stix and then headed to Sean and Brooke's in HB. We went down to the beach and played, while Kyle took Gavin and Parker into the waves and Gavin caught his first bodysurf wave all alone. This kid is becoming quite the fish. And Parker also enjoyed swimming under the waves with Dad. Then we went out to dinner to a yummy mexican restaurant. It was fun to catch up and the boys always love their cousin time with Finn.

4th of July

We had such a fun 4th. It started in Hunington Beach with Kyle's brother Sean, his wife Brooke and their son Finn. We saw their annual parade, which was amazing, and then explored the cute downtown and had lunch. It was fun to catch up and the boys loved their cousin time.

20 weeks pregnant. This thing is growing!

Here HE is! Our third boy. :) It was not much of a surprise...I knew we were going to have another boy, but the excitement of seeing your baby for the first "real" time was amazing. Even though this is not my first rodeo, I was more emotional and excited then I was with the Gavin and Parker. I love my boys and can't wait to add another to the gang.

The boys are so excited about Mommy having another baby, and it is becoming more real as my belly continues to grow; however, they really wanted a sister. Gavin cried, really cried, when he found out he was having another brother, and Parker keeps calling the baby a she. But, I know they will love this baby and accept him into their elite clan.

I love my boys so much! It is true... sometimes they drive me crazy, and I want to pull my hair out and scream, but right now as they are resting in their beds and the house is quiet, I miss them. I know, weird! I always knew having kids would be fun and amazing. I remember when we first got married I would long for the day when my car would be filled with car seats and rowdy kids. I remember watching a mother with her kids at the grocery store loosing her cool and thinking that would never be me. Hahaha! Motherhood is everything I longed for and more. When I watch my kids play I sometimes can not believe they are mine. How was I blessed with such amazing little people? What did I do to earn such a blessing? I know I take them for granted and all too often I lose my temper and do things I wish I wouldn't. I know motherhood is the toughest job in the world with too little appreciation. I know one day I will miss these moments where they are carefree, without any scars from the world. I want to be better. I want to have no regrets, and so I promise I will do all I can to love them and care for them as I would want to be cared for. I promise to cherish the moments as if they are my last, and I promise to stop what I want to do for my selfish self and think of them. I love being a mom and hope to be all I should be for them.

I love free Tuesdays at the museums. The boys love the Natural History museum with the dinosaurs and now they have snakes and lizards there too. Needless to say, it is a boys dream. It was a beautiful day so we went to play at the huge park in Balboa Park. Great afternoon with my little me.

Walk to Grandma and Papa's house

Gavin has been asking to walk to Grandma's house for a few months now, and so we finally did. The weather was a little chilly and the rain was to come any time so we walked quick, and beat the rain by just a few hours. It is about a 3 mile walk and pretty hilly, especially hard when you are pushing a 25 lbs. stroller, and two 3o+ lbs. boys. But all in all it was a lot of fun!

It was a rainy Saturday and Kyle was working so we decided, since Mommy doesn't have to teach on Saturdays anymore, to play! It was so fun. We stayed for 3 hours and loved every second of it, even Mommy.

Valentine's Day Treat

For Family Home Evening, every Monday night, we do a fun craft and lesson either about Jesus, another important person or we talk about Christlike attributes. Since this FHE was on Valentine's Day we talked about loving one another... and made a heart-shaped brownie to go with the lesson. :) So fun. The boys look forward to this every week.

Why I want to be a Fireman?

Kyle had this open and I love it. He is in the process of applying for a Firefighter Paramedic job and has an interview next week. He has been studying and polishing up his "script" for the interview. Firefighter interviews consist of 30 possible questions, and since the interview is weighted so heavily, applicants really get their 30 answers perfect. I loved this answer to "why do you want to be a firefighter?" Kyle has such a way of expressing himself and is extremely personable. I have all the faith in him and I know he will do great!

In 2002 I started as an employee for a large financial institution. Over the years I worked my way up and was able to make a great wage, invest in a pension, 401k, real estate and other endeavors. It was an exciting time for my growing family. In 2008, The real estate market came crashing down devastating many small businesses, large corporations and massive financial institutions. One day I had a flourishing career with a bright future and the next I was hit with a tidal wave of despair and fear. I remember the day I had to go home and tell my wife that everything we had earned, due to sacrifices and hard work over the past years, were gone. When our savings ran out, we had to cash out all of our investments, 401k and pension to try and pay off the mounting debt we began to accumulate to stay afloat. I was devastated. I felt like I let my family down. My wife looked at me and smiled, she asked, “what have you always wanted to do with your life?” I said, “well, If I could do it over I would be a firefighter, but we have a family, we can’t afford it.” She replied, “either way we have to start over. We will figure it out. So quit crying and go get it.”

Since that day I have been on a sprint to learn as much as I can and get as much fire and EMS experience as possible. I absolutely love it. I would not change a thing. I am so happy to have gone through those hard times to be able to enter this new adventure. To me, finding this path is worth more than gold.

I want to spend the rest of my working days in the service to others, 100% committed to a worthy cause. I don’t want to waste my life only focused on building a fat bank account or a large investment portfolio, I want to be a shining example to my children of hard work, respect and selfless service. I want them to grow up seeing others who believe the same.

Since my last tough conversation years ago with my wife about losing our lively hood, I have been waiting for the day when I can go home, sit her down and tell her that all of her commitment, sacrifice and faith in me has paid off and our dreams of becoming a firefighter family have come true.

Parker

My sweet Parker (almost 3)! This kid seriously melts my heart. He could honestly get away with murder. His sweet squeaky voice, big blue eyes, dimples and rabbit teeth just gets right to me. I know I probably baby him and spoil him, but until I have another baby he will have to do. He is such a snuggle bug and loves to have his "lovie" (satin blanket) with him at all times, but don't worry, if he forgot it any satin will do. He loves his tags inside his shirt and especially Daddy's "lovie shorts" (his work-out shorts). He is a little bit of a neat freak and constantly reminds me to vacuum...as he pulls out his to accompany mine. He is obsessed with skateboarding and has just started to ride primarily on his feet, no more knee boarding for Parker. On Friday nights in Poway it is "Kids Only" skateboarding. He felt the peer pressure and really stepped up his A game; he stood up and rolled down the hill. A super big deal! He loves his big brother and his Mommy and Daddy and we sure love him too!

Gavin

Mr. Gavin (4 1/2)! My little man with his deep voice and mature attitude. He is our natural athlete. This kid is good at anything physical. He loves to dance; hip hop and break dance as well as wrestling and rock climbing. He forgets that most kids don't like to play as rough as him and sometimes plays too rough at preschool. Ooops! He loves to learn and actually read his first sentence today! He is such a great helper and plays pretty well with Parker. He is Daddy's sidekick and always wants to help him with anything, even study his paramedic books with all the gross graphic pictures. He really loves to go to the skateboard park with Daddy and is shredding the "gnar". Everyday he changes his future profession, but has remained consistent with a career in service. He definitely has been the perfect first child and I know he will be an amazing example to his siblings. We love this busy boy and could not have picked a better kid!

Trick-or-Treat

Skeleton and a Giraffe. We went trick-or-treating at Grandma Michele and Papa Joe's house. It was so fun. The boys wanted to trick-or-treat all night, but good thing their pails were small and filled up quickly!

We went over to my Mom's house and made Black Cat Cupcakes, Spiderweb Pizza, and carved one last pumpkin.

W had such a fun time going to Bates Nut Farm with our friends the Carpenter's and their cute girls. The boys had a blast running around, climbing huge walls, riding pones and playing with their friends. Overall a great day.

Arizona Roadtrip

We took a roadtrip with Kyle's family for Brenna's Irish Dance Competition. It was a fun excuse to get a way and play! Brenna did awesome and we are all so proud of her and her fast feet! If you have seen the movie "Little Miss Sunshine" it was totally like that!

We went to an abandoned old gold mining town and played. They had a mining tour, fun shops and a really tall cactus.

Papa Tony

My Grandpa...Papa Tony is such a playful, silly Papa to my boys and has always been an amazing Grandpa to me. He was the grandpa who always gave you cookies and candy and made you feel special. He has had a rough year and has become a little different and at times it has been very difficult to see him this way. He suffers from Alzheimer's Disease. It has been difficult to visit him in a care home and have to remind him who you are. Along with his Alzheimer's he also had a stroke this year causing him to mumble and have a difficult time speaking. We love him so much and really cherish the memories of who he was. This experience has helped me to value family more and enjoy the time you have with one another; and to especially make time for each other. I love you Grandpa and thank you for being so loving and kind.

My Grandma and Grandpa at my Grandma's 90th Birthday party. So grateful for this party that brought us all together. The next month his health went down hill. Thanks to my mom for having this party and giving us great photos to cherish.

We try to visit him once a week at the care home. The boys have been amazing! They often bring tears to my eyes at their display of compassion. They run up to Papa Tony and give him hugs and kisses with out a single prompt from me. They talk to all the other Alzheimer patients and love them as well. I always leave so proud of them and their love they show to others. Papa Tony does not recognize them and often does not even talk to them, but he is full of smiles when he sees them. I know it brightens up his week when they visit. (took pictures with my phone...not the best quality)

This little old lady talking to Parker runs up to us when we visit and insists that my sons are her boy friends. It is pretty funny the conversations we have with other patients. It is definitely entertaining. :)

Happy birthday Gavin! It is soooo hard to believe my baby is 4, I know I say that every time they have another birthday, but it is going by too fast. :( Gavin is my little book worm...hence the theme for the party, "The Cat in the Hat". It was such a fun theme and allowed a lot of creativity. He had a great time, and so did we.

Thanks Gavin for being such an awesome little boy. Kyle and I often look at him (Parker too) and are so in awe of him. He is a super funny little boy, a great big brother (most of the time), a loving friend, super social and friendly, this kid will talk to anyone...a little scary, I know. We have had the stranger talk sooo many times, and he still has no fear. He is a smart little boy. He loves to read and learn. He has sooo many questions for everything and anything. If Gavin could pick his perfect day it would be: wake-up at 7am (he has an alarm clock inside of him, I swear...7am on the dot, everyday), magic pancakes for breakfast, watch Super Why on PBS Kids, read books for as long as he wants, paint outside, peanut butter and honey sandwich for lunch, more books, more books, more books, skate park with Daddy, pizza for dinner and frozen yogurt for dessert, more books for bed time and a prayer and he is down for the count. I feel so blessed to have him as my son, and look at him and ask myself, "is this really my son?" He has truly melted my heart and I thank God for him each and every day. Happy Birthday Gav, Mommy loves you!

Road trip to Puget Sound! Wahoo! You would think I was crazy, in the car with a 2 year old and 3 year old for 24 hours, but it was awesome. The boys were incredible, almost perfect...really! I was expecting the worst, and they did great. I had a reward bag for them, and every couple hours if they were good they could open a present. The gifts ranged from masks, hats, coloring books, puzzles, treats, etc. It worked, they wanted to open a present.

So as you can see from the pictures I traveled with Kyle's family without him. He is in the middle to end of his internship for Paramedic School and could not be gone for 11 days. The boys and I had so much fun, but missed Kyle like crazy, and he really missed us.

We stayed at this amazing beach house on Puget Sound. It was Michele's (Kyle's Mom) cousin. It was incredible, seriously out of a home decorating magazine. Gavin and Parker loved to walk along the water and throw rocks, roast marshmallows and hot dogs over the fire, go on the boat, and so much more. It was a great trip and now I know my kids can travel!

We stayed in Washington for 5 days then headed back down. We got dropped off at my sister Jen's house in Redding and hung-out with them for 5 days. Too much fun. We went to the lake, pool and enjoyed each other. Such a great break from the day to day routine, and really special to see everyone.

My Grandma's 90th Birthday

My Mom's Mom, my Grandma Mickey, turned 90. Crazy! If you met her you would never know. She is just as spunky as she always has been. That is why we love her; her tell it like it is comments and always down for a party attitude. The party was a total surprise which made it much more special. She had her 4 living sisters come from New York, Miami, and 2 from Puerto Rico. As well as cousins, nephews, and nieces from all over, and of course life long friends. My mom put on this event and it turned out to be a weekend I, as well as everyone involved, will never forget. It truly became a family reunion. I met many relatives I have never met before. It was amazing, and so special for my Grandma.

My Grandma is one of 18 kids, and she is number 9. She was raised in Puerto Rico and came to New York City when she was 18. She is the most driven person I know. She will tell anyone the story of her first 2 weeks in the states, so I will try, it goes something like this: She came all alone with only a few dollars to her name, and unable to speak English. She had to look for a job quickly because her money was going to go. She only had a loaf of bread and a gallon of milk to last her until her first pay check (in 2 weeks). She was hungry and lonely. She made it. She told herself that she was going to be someone and never be hungry again. And she accomplished that goal. She met my Grandpa Tony who had a similar story. He came here from Puerto Rico around the same age. They met dancing and began their life together (now married 64 years). They had my mom and uncle in New York, but then decided they wanted to move to LA to open a restaurant, and they did. They opened up a few restaurants and became very successful. My grandparents are shining examples of the American Dream. I know that I can do and become anyone I want to because of what they accomplished. I am so blessed to have them in my life and to learn from them. I know as they continue to age my time with them is limited, and that breaks my heart. I am so guilty of not calling them or visiting them as often as I should. But I know one thing for sure, I love them so much and truly brag about them often. Happy 90th Birthday Grandma Mickey, I love you so much.

Happy Birthday Parker! I can hardly believe he is 2! Where has the time gone by? He is such a busy boy who loves to go outside and run around, and so sweet, always full of kisses and hugs. He especially loves his big brother Gavin and loves to play with him and laugh at all his silliness. He has begun to talk a ton all of a sudden and now does not stop. Parker Weekend!....No Gavin

Gavin went on a trip with my parents to Redding, CA to visit my sisters family for my nephew Addison's 4th Birthday....Gavin's BFF! So we had Parker all alone, so fun and so easy. No timeouts, fights or cries. One kid is cake! We decided after a busy week, as usual, to enjoy the warm weather on a Saturday afternoon and go to La Jolla Cove. It was a blast.

The end. The boys loved Disneyland, and we did too. We got there when it opened and left when it closed, enough said. My baby is now in a big boy bed! I know I probably jump the gun and transition my kids into a big boy bed early (Gavin in a big bed at 18 months, Parker at 22 months), but I hate cribs! Changing the crib sheet is the worse.....So Parker is now officially a BIG boy. He loves his bed, I think mostly for the freedom and because him and Gavin have the same bed. (I will show pictures of their room when I am done with it!). Every morning at 3:00am he comes into our bed. Kinda annoying, but at least we do not have to get a crying, crib bound baby! We went to the doctor on Friday and these are his stats: 24 lb. 13 oz., 34.5 inches. Tall and skinny!

Parker is probably the cutest, sweetest kid! He is a snuggler and quite the little talker, as of a few weeks ago. He is talking up a storm, and is so proud of himself. We love him so much and are kind of sad he will be 2 next month. Boohoo...

Halloween this year was eventful! On Friday the 30th we went to Kidsville in Poway to their Halloween Party. They had trick-or-treating, crafts and a performance by the Hullabaloos. The kids loved it. Then on the 31st we went to my parents house, a tradition, to eat chili and pumpkin pie then hit the street for some candy. The kids really got the hang of trick-or-treating and filled up their buckets in no time. Parker was very serious and would not let me help hold his heavy candy bucket...afraid I would take it. We love Halloween and can't wait until next year.

Pumpkin Carving

We had fun carving the pumpkin into a "scary gorilla", a skull. (Gavin as a baby called skulls scary gorillas) Gavin picked one of the hardest outlines, so I did most of it.

We went to Bates Nut Farm, like usual! It is always a blast. We went on pony rides, pet animals in the petting zoo, picked out a pumpkin, enjoyed a wheelbarrow ride from Daddy and ate yummy salt water taffy! Now we just need to carve the pumpkin! :)

Happy 3rd Birthday Gavin!

We had such a great birthday! Gavin wanted a monster theme. It was a little tricky, but it came out great. We had a red frozen monster hand in the punch bowl, monster hands (clear plastic gloves full of popcorn and Dot candies for finger nails, monster cupcakes and monster masks. The weather was out of control (104 at 3:00pm), poor kids!

I can not believe my baby is already 3. His favorite line is, "Mom I am big now, but I still love you." Thanks bud! We love him so much and he really brings a lot of smiles into our home.

Another of Kyle's Paramedic ride-a-long experiences. He has to write this mini paper for his ParamedicSchool. And as usual I edited it, so I figured I could steal it! I hope he does not care. I like hearing about all of his amazing experiences and watching his face light up with excitement as he shares them with me. We are on a interesting journey we never thought we would have, but we are enjoying it and all the mountains we are climbing to get to where we want to be. Enjoy. He wrote this very quickly, so it is not his best work, but it is great to read!

The pre hospital setting can be a scary place for a paramedic. To give good patient care and stay safe while making thoughtful decisions can be very intimidating, to say the least. There was a specific call that I had on one of my ride alongs that opened my eyes to a whole new side of para-medicine. Not because it was bloody, gory or overly exciting, but because it involved a very important, split second decision.

We arrived on scene to a gas station at about 2 am. Fire and PD were on the scene. There appeared to be only one patient with two other bystanders being questioned by the PD. The patient was standing with the fire fighter who was holding a manual c-spine. Every few seconds the patient would start to squirm and try to get away. The fire fighter would calm him and let him know he needed to hold still for his safety. It turns out that the patient was struck in the back of the head and hit his face on a car before hitting the ground and losing consciousness. The patient was not being cooperative and did not want to go to the hospital. “Who’s gonna pay my hospital bills!” He yelled over and over again. The paramedics tried to explain to him that he could have something seriously wrong with him and it would be foolish to worry about the money. It sounded to me as though this patient, being in his late twenties, was going to AMA. I was wrong. The paramedic began to ask orientation questions. “What is your name?” The patient answered correctly. “What city are we in?” “uuuuuuuuum, uuuuuuuuum San Diego?” The patient answered slowly and with confusion. The paramedic looked at me with a half smile and asked the last question. “What year is it?” “I don’t know.” answered the patient. Apparently, that was all the medics needed to hear. No one said a word. The medics looked at the firefighters as though they were all thinking the same thing. “ This guy is altered and can’t make decisions for himself.” With 2 firefighters and 2 medics the patient was carefully forced to the gurney. He was extremely upset, to put it lightly. Lucky for us, the patient was calmed during transport and ended up being more pleasant then a problem.

The idea of using force to make someone go to the hospital is new to me. I felt like we were taking away someone’s right to refuse care. Having said that, I do understand that this man had serious neurological deficits that could cause very long term damage. The paramedics had three facts to base their decision on. This patient was obviously altered, he had lost consciousness and he had a bleeding laceration to his forehead with a hematoma on the back of his head. When I looked at it with the facts in mind, I quickly changed my view. He needed to go to the hospital and he was in no condition to make that decision for himself at the time.

This was an example of a very important lesson learned on a ride along. This is proof to me of the importance of doing as many ride alongs as possible. If I run into this type of call in the future I will know what to do. With out experiencing this 1st hand before my internship, I don’t think I would have had the confidence to restrain this patient. You can read the protocol book all day but using what it says in the field in split second decisions can be quite intimidating.
